Appointments are available Monday through Friday. To schedule an appointment, please call the office location that is most convenient for you.
Update your patient health history form in our office
You will only need to complete the items that have changed, including medications, medical allergies, preferred pharmacy, address, phone number(s), e‑mail address, etc.
Please arrive 20 minutes before your scheduled appointment and bring the following items with you:
- Current insurance card.
- Current list of medications including prescriptions, over-the-counter medications, and herbal supplements.
- Your co-pay. We accept cash, checks, VISA, MasterCard and American Express. We will submit your claims to your insurance company, but co-pays must be made at the time of your office visit.
