Returning Patient : Download Update Forms
You have chosen to update your patient health history by downloading forms that you will bring with you to your appointment. Please download the ENT Patient Health History Packet, update the forms at your leisure, and bring them with you to your appointment.
You only need to complete the items that have changed, including medications, medical allergies, preferred pharmacy, address, phone number(s), e‑mail address, etc.
Please arrive 10 minutes before your scheduled appointment and bring the following items with you:
- Updated ENT Patient Health History Packet.
- Current insurance card.
- Your co-pay. We accept cash, checks, VISA, MasterCard and American Express. We will submit your claims to your insurance company, but co-pays must be made at the time of your office visit.
If needed, please obtain a referral authorization number from your insurance company prior to scheduling your appointment.
