New Patient : Forms in Office
You have chosen to complete your Patient Registration Forms in our office.
Please arrive 30 minutes before your scheduled appointment and bring the following items with you:
- Current insurance card.
- Current list of medications including prescriptions, over-the-counter medications, and herbal supplements.
- Your co-pay. We accept cash, checks, VISA, MasterCard and American Express. We will submit your claims to your insurance company, but co-pays must be made at the time of your office visit.
If needed, please obtain a referral authorization number from your insurance company prior to scheduling your appointment.
