You have chosen to complete your registration forms before your appointment. Please download the ENT Patient Health History Packet and the Privacy Policy, complete the forms at your leisure, and bring them with you to your first appointment.
Please arrive 20 minutes before your scheduled appointment and bring the following items with you:
- Completed ENT Patient Health History Packet & Privacy Policy Statement.
- Current insurance card.
- Your co-pay. We accept cash, checks, VISA, MasterCard and American Express. We will submit your claims to your insurance company, but co-pays must be made at the time of your office visit.
If needed, please obtain a referral authorization number from your insurance company prior to scheduling your appointment.
