Returning Patient : Online Patient Portal

Thank you for choosing to update your information on our secure HIPAA-compliant patient portal before your appointment.

You will only need to update the items that have changed, including medications, medical allergies, preferred pharmacy, address, phone number(s), etc.

Please arrive 5 minutes before your scheduled appointment and bring the following items with you:

  1. Current insurance card.
  2. Your co-pay.  We accept cash, checks, VISA, MasterCard and American Express.  We will submit your claims to your insurance company, but co-pays must be made at the time of your office visit.

If needed, please obtain a referral authorization number from your insurance company prior to scheduling your appointment.
